Drive a new 3 16 by 1 inch concrete anchor into each of the holes with a hammer. Lift the threshold out and drill 1 1 2 inch deep holes at each drill tip mark on the concrete. Remove dirt and debris from chips and hairline cracks in the concrete threshold using a stiff bristle cleaning brush. Position the old door sill on the new sill and trace around it. Install a cover alone or purchase a cover and sill combination. The aluminum door threshold should sit level on the sill and join to it with a waterproof flexible and adhesive sealant polyurethane. Remove all debris from the concrete slab.

Lay a bead of caulk along what will be the back edge of the threshold. Hold the new door sill in place with construction adhesive and fasteners.
